This was a custom sculpture project for a cosplayer wanting to make a cosplay of Puppetmon (from Digimon). Yes, in Japanese, it was Pinocchiomon.
This full head sculpture had a separate jaw and 3 separate nose sections.  The nose was assembled with a wire core to help keep it standing straight. The jaw was put onto a fabric hood to allow for movement and articulation. All parts were made in cold foam latex with a slush cast latex skin. The whole mask was painted up with pax paints to match the character.
